Home Together turns the scattered notes, calendars, and family texts into a plan everyone can understand and update.
Keep appointments, home routines, rides, and next steps together so the family can see what is coming.
Write down who is helping and when, share practical visit notes, and make the backup plan easier to find.
Track meals, shopping, laundry, pet care, household needs, and the small things that keep a day on track.
Create a clear, large-print care workbook to put on the counter or take along to an appointment.
Reuse daily and weekly checklists instead of rebuilding the same plan every time.
Keep family notes and practical handoffs together. Home Together helps organize care; it does not replace clinicians or emergency services.
You do not need to set up everything in one sitting. Add the next appointment, the next visit, or the one reminder that keeps getting lost. Build the plan as your family needs it.
